Claim Notification

In order to facilitate the claim notification procedure we have set up a number of basic rules for you. Provided that you observe them you shall avoid many inconveniences and thus you shall speed up the whole claim settlement process. Besides, on this site you can find the appropriate claim notification forms to be downloaded, filled in and forwarded by you to the Insurer's address.

Claim notification procedure

Accident

• After the treatment shall have been finished you should forthwith notify the Insurer of the claim.
• Ask your insurance counsellor for Accident Notification Form. You can equally ask the call centre staff to send it to you, or you can download it on this site.
• Fill in the first form page carefully; do not forget signature of the Insured or that of the Beneficiary. In the form, give all and any life and accident assurance policy you have made with QBE. If you wish to send benefits to your bank account write down its number legibly.
• Ask your attending doctor to fill out and confirm overleaf of the form after the treatment of accident is finished.
• Send the correctly filled out and signed form to the address: QBE poisťovňa, a.s., Štúrova 27, 042 80 Košice
• In order to handle your claim promptly you should equally attach copies of medical findings concerning your accident (including actual findings after the end of treatment)

A claim from property & liability insurance

• Notify the Insurer forthwith of the claim.
• Consult the claim staff which form should be filled out and which documents you should present for claim handling. Agree the survey date with the claim settlement staff. You can download the claim notification form here. Fill out the form carefully and attach thereto all necessary documents. Do not forget to sign the form.

A claim from motor insurance

In the event of car accident the party to car accident is liable to observe the provisions of Articles 50 and 51 of the Road Traffic Act 315/1996.

• Report each claim to the Police. Upon failure of this condition the Insurer is authorized to reduce indemnity proportionally.
• Note down the identification data of any party to and/or witness of car accident.
• Advise the Insurer forthwith in writing of claim occurrence.
• The Insurer´s loss adjustor shall contact the Insured for motor vehicle survey. After the vehicle damage is recorded the Insured shall present a copy of such a record to the garage that shall repair the damaged vehicle. If garage reveals other defects the Insured is liable to agree an additional survey with loss adjustor (or authorize the garage to work with the Insurer).
• The condition of the damaged vehicle as a result of insurance event must not be changed before the first survey is accomplished, except for necessary safety, sanitary or other substantial reasons.
• If the vehicle must be towed the Insurer shall pay the reasonable expenses of towing the vehicle from the place of accident to the nearest branded garage. The loss adjustor shall ensure the survey direct in garage.

CAR ACCIDENT ABROAD

• Call up the Police each time.
• Sign the record provided that you understand it and agree with it. Otherwise, you should give your statement in Slovak, regarding the car accident.
• Note down the names, vehicles registration numbers and other identification data of the parties to and/or witnesses of the car accident, as well as the Insurer of offender´s motor third party liability insurance.
• If motor vehicle must be towed then the Insurer shall pay the reasonable expenses of towing the damaged vehicle from the place of car accident to the nearest branded garage in Slovakia .
• If vehicle must be repaired abroad the Insurer shall pay the necessary costs of making the vehicle drivable. The remaining defects, if repaired abroad, shall be reimbursed in prices as applicable in Slovakia .
